POULSBO — The state Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our area.
— In July, the Department of Health suspended the credential of registered nurse assistant Jessica Nicol Cody (NA60061582) for six months. Cody slapped a vulnerable adult client.
If Cody completes agreed requirements, she may apply for credential reinstatement, and will be placed on probation for at least 18 months from the date her license is reinstated.
— In May, the Chemical Dependency Professional Program granted the credential application of chemical dependency professional Krissy Merrion Remeto (CP60246706).
Remeto must comply with terms and conditions set against her credential.
— In April, the Dental Commission charged dental assistant Teresa Marie Brakefield (D160028485) with unprofessional conduct.
Brakefield was allegedly fired from two dental office jobs for being under the influence at work, and for repeated prescription fraud and substance abuse.
