OLYMPIA — The state Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in Kitsap County.
In July, the Secretary of Health denied provisional certified home care aide and certified home care aide credentials to Jennifer Lynn Handke (PV60602634, HM60602632). On her application, Handke reported receiving intensive inpatient and outpatient treatment at a recovery center.
In September, the Dental Commission entered an agreement with dentist Hamidreza Barakhshan (DE00009257) that fines him $1,000 and orders him to reimburse the commission for $4,268 in expenses. Barakhshan must complete continuing education in wisdom tooth extraction, radiography, dental anesthesia, and recordkeeping. Barakhshan fractured a patient’s jaw, damaged a tooth’s root, and didn’t keep adequate records.
In September, the Nursing Commission reinstated the registered nurse credential of Monica Celeste Nettles (RN60428058), whose Mississippi nursing license was suspended in November 2015.
In September, the Chiropractic Commission ended conditions on the credential of chiropractor Blake J. Wagner (CH00003095).

The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally may call that number and report their complaint.
