PORT ORCHARD — Kitsap Public Health District and Harrison Medical Center is seeking input through a brief survey about the quality of life and community health in Kitsap County.

Survey information will be combined with health data to assess opportunities to improve health and well-being in Kitsap County, said Karen McKay Bevers, public information officer for the health district.

Bevers said the survey — accessed at www.surveymonkey.com/r/HarrisonsSurvey) should take about five minutes to complete. She also said the survey is anonymous.
